The patient is responsible for facility fees (operating room use, supplies, medication, implants), applicable copayments, coinsurance, and/or deductible payments on or before the day of surgery. For your convenience, Covenant High Plains Surgery Center accepts cash, checks, debit cards, and credit cards (Visa, MasterCard, Discover, American Express, and CareCredit).
Please be advised, all fees are estimations. If there are any additional charges or fees, the patient may receive a statement after insurance has processed the claim. The patient may receive a separate statement from other facilities or doctors offices regarding lab, pathology, EKG fees and/or NorthStar Anesthesia or other anesthesia providers regarding anesthesia fees.

Prior to your surgery, our insurance specialist will contact you to discuss payment of fees and answer any insurance questions you may have. Because insurers often prefer to work with ambulatory surgery centers, Covenant High Plains Surgery Center participates in most insurance plans. If we are not in network with your insurance, as a courtesy to our patients, we will file the claim. The patient is responsible for any applicable out of network copayments, coinsurance, and/or deductible payments.
Our insurance participation is continually changing, so we encourage you to call us with insurance questions and to confirm that we are within your insurance network.
