Insurance and Billing

Covenant High Plains Surgery Center provides our patients with the best service and quality care. The patient is responsible for the Facility Fees which is comprised of the use of one of the operating rooms, supplies, medication, and implants (screws, anchors, pain pumps, etc.) The patient is responsible for any applicable co-payments, co-insurance, and/or deductible payments on or before the day of surgery. Our insurance specialist will contact you in advance to discuss payment of these items and any other insurance questions you may have. Please be advised, all fees are estimations. If there are any additional charges or fees, the patient may receive a statement after the insurance has processed the claim.

The patients’ surgeon’s fee, anesthesia fees, and charges for any related laboratory work, pathology, and EKG fees are separate from the facility fee.

The Patient may receive a separate statement from other facilities regarding lab, pathology, EKG fees and/or NorthStar Anesthesia or other anesthesia providers regarding anesthesia fees.

For your ease, Covenant High Plains Surgery Center accepts cash, checks, debit cards, and credit cards (Visa, MasterCard, Discover, and American Express).

Covenant High Plains Surgery Center participates in most insurance plans. If we are not in network with your insurance, as a courtesy to our patients, we will file the claim. The patient is responsible for any applicable out of network co-payments, co-insurance, and/or deductible payments.
